This authorization to institute a class action is aimed at all Quebecers who have resided in a CHSLD since July 11 2015, in conditions that are contrary to the Act Respecting Health Services and Social Services.
The potential violations of the law are enumerated in the following form which we invite all concerned persons to fill out :
Larochelle Avocats is currently gathering information regarding the fulfillment of Quebec’s residential and long-term care centres’ obligation to provide a proper alternative environment to users who have lived there since July 10th, 2015.
To assist us in gathering information, we invite you to fill out a questionnaire. This questionnaire was designed to obtain more information on the quality of daily care and services received by users of Quebec’s residential and long-term care centres since July 10th, 2015. The information you will share with us will be considered strictly confidential and won’t be shared with anyone.

Notice to members of April 17, 2021
On 24 March 2021, the Superior Court approved the final version of the Notice to Class Members to be published on 17 April 2021. Class members are invited to read the Integral and abridged of this Notice, which contain important information on the present Class Action.
These Notices contain, inter alia, information regarding the persons included in the Class Action, and on the opting out procedure, which deadline has been set up for 28 June 2021. Further, those who have not yet filled up the form available on the Class Action website are invited to do so in order to provide their coordinates to the Class Actions counsels.
In view of the difficulties arising from the COVID-19 Pandemic, the counsels representing the Class members will not be able to visit CHSLD to gather information from the Class members about their situation. For this reason, Class members who would like to share such information are invited to fill the form available on this web site.
